Class Liability Waiver Form

RELEASE/WAIVER: I hereby agree to indemnify and hold harmless Moonlight & Mindfulness and its employees at 91 Mill St, Unit 7, Dracut, MA 01826 against all claims for personal injuries or damages of any kind arising from participation in any Candle-Making workshop, specialty classes, and their resulting products.

 

*NOT MEDICAL ADVICE: I understand that no ingredients, ingredient descriptions, or guidelines presented are intended to treat, prevent, or cure any disease or condition.

 

*PHOTO CONSENT: By your signature, you agree that Moonlight & Mindfulness may use the below named participants photograph in the routine promotion of its activities.

 

*SUBSEQUENT VISITS: This waiver shall be kept on file and adhered to for both initial and subsequent visits to the workshop by the participant.You may revoke this waiver at any time by sending a statement via certified mail to: Moonlight & Mindfulness, 91 Mill St, Unit 7, Dracut, MA 01826. I fully understand and acknowledge that the workshops offered at Moonlight & Mindfulness may involve inherent risks, dangers and hazards which may result in injury. All Candle-Making Classes & Workshops offer a No Refund Policy.

 

I hereby voluntarily agree to release, waive, discharge, hold harmless, defend, and indemnify Moonlight & Mindfulness, its managers, members, and employees from all claims, actions or losses for bodily injury, property damage, wrongful death, or otherwise, which may arise from my participation in any workshops or classes, resulting products from workshops or classes, or from use of equipment provided by Moonlight & Mindfulness.I accept full liability for creations made by participating in the workshop.
